Job Summary

We are seeking a compassionate and skilled Social Worker to join our team at Your Health Organization. As a Social Worker, you will play a vital role in providing exceptional care to our patients and their families in the comfort of their own homes.

By going beyond medical treatments and assisting patients with their additional needs, we create a comprehensive and nurturing environment.

Key Responsibilities
  • Initiate Advance Care Planning for patients throughout their assigned Care Team (including facilities).
  • Complete Daily Comprehensive Assessments on patients.
  • Complete Daily Care Plans on patients.
  • Document Chronic Care Management time daily.
  • Meet Daily Productivity Goals
  • Coordinate discharges at contracted facilities
  • Coordinate Placement.
  • Continuation of coverage (i.e. coordinating acute visits for falls, med changes, etc.)
  • Build and maintain professional relationships with referral sources.
  • Maintain strong communication and professionalism between referral sources, patients/families, and Care Teams.
