Job Summary
We are seeking a highly skilled LPN to join our team as a Critical Care Nurse. As a Critical Care Nurse, you will be responsible for providing high-quality patient care in a home setting. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities
- Provide high-quality patient care in a home setting, including administering medications, performing wound care, and providing emotional support to patients and their families.
- Develop and implement individualized care plans that meet the unique needs of each patient, in collaboration with our interdisciplinary team.
- Conduct thorough assessments of patients' physical and emotional needs, and document findings in a clear and concise manner.
- Collaborate with our team of healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Participate in ongoing education and training to stay up-to-date on the latest best practices and technologies in critical care nursing.
Requirements
- Current LPN licensure in good standing.
- Valid CPR certification.
- Graduation from an accredited and approved nursing program.
- Minimum of 16 hours of availability per week.
- Ability to travel within Essex and Union County.
