Job SummaryThe successful candidate will be responsible for evaluating physician medication orders, making recommendations for therapy changes, and collaborating with the healthcare team to provide clinical and technical support. This role requires a strong understanding of medication management, patient care, and regulatory compliance.
Responsibilities- Maintain accurate patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication.
- Ensure laboratory tests are ordered, evaluated, and discussed with the healthcare team as needed.
- Collaborate with the healthcare team to provide clinical and technical support.
- Document all communication with physicians, nurses, and other healthcare team members in the medical record.
- Implement policies and procedures for safe medication management, procurement, storage, distribution, and disposal.
- Obtain and maintain required documentation in accordance with federal and state regulatory agencies and policy.
- Inform the Pharmacy Manager of medication-related errors.
- Assist facility staff with procedures related to safe medication management.
- Bachelor's Degree in Pharmacy required; PharmD preferred.
- 1+ year of pharmacist experience, preferably in Long-Term Care.
- Must be licensed as a pharmacist in the state of PA.
