We are seeking a talented Registered Dietitian to join our team at NYC Health + Hospitals Certified Home Health Agency. As a key member of our interdisciplinary team, you will be responsible for providing expert nutrition care to patients in their homes.

Responsibilities include conducting comprehensive nutritional assessments, developing personalized care plans, and educating patients and their families on healthy eating habits and disease management. You will also collaborate with healthcare teams to ensure seamless patient care and coordinate services.

A competitive salary of $75,000 - $95,000 per year, based on experience, is offered, along with a comprehensive benefits package that includes health insurance, retirement savings and pension plans, loan forgiveness programs, paid holidays and vacation time, college tuition discounts, professional development opportunities, and multiple employee discounts programs.

About the Position:

  • Assess patients' nutritional needs and develop individualized care plans.
  • Provide education and counseling on healthy eating habits and disease management.
  • Collaborate with healthcare teams to ensure seamless patient care and coordinate services.
  • Maintain accurate and detailed records of patient interactions and progress.

Qualifications:

  • New York State License as a Registered Dietician.
  • Current registration with NYS Department of Education in Nutrition.
  • Current certification in Basic Life Support procedures including CPR.
  • Bachelor's Degree or higher in dietetics or related program approved by the Academy of Nutrition and Dietetics.
