Nurse Practitioner

We are seeking an experienced and compassionate Registered Nurse to provide individualized care to adult and geriatric clients in the comfort of their own homes.

• Assess clients' needs, developing and implementing personalized care plans that address their unique requirements.
• Deliver high-quality patient care, adhering to established protocols and best practices.
• Monitor client conditions, identifying potential risks or concerns, and communicating with healthcare teams accordingly.
• Document all interactions, interventions, and outcomes accurately and promptly.
• Collaborate with healthcare professionals to ensure seamless care coordination and address any concerns or changes in client conditions.

To excel in this role, you must possess:

• A current Registered Nurse license in Pennsylvania.
• At least one year of recent clinical experience, preferably in a medical-surgical setting.
• Prior home care experience is highly valued but not mandatory.
• Graduation from an accredited nursing program.
• Exceptional communication and interpersonal skills, with the ability to work independently and prioritize tasks effectively.
• Strong computer skills, preferably with experience using electronic medical records.

The estimated annual salary for this position is $80,000-$100,000, commensurate with experience. Our competitive benefits package includes paid holidays, vacation, sick leave, vision, dental, and medical health plans, and a 401(k) with company match.
