We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Country Meadows Retirement Communities. As an LPN, you will play a vital role in providing high-quality care to our residents and contributing to our mission of making lives better.
Key Responsibilities- Form strong relationships with residents, their loved ones, and physicians to develop personalized care plans.
- Coach and support colleagues to ensure seamless care delivery.
- Monitor medications, oversee physician orders, and round with physicians to ensure quality care.
- Educate residents and their families on health and wellness topics.
- Assume responsibilities as Manager on Duty and share leadership responsibilities.
- LPN license in the state of employment.
- CPR certification (Country Meadows will provide certification if needed).
- Minimum one year of experience in assisted living or healthcare setting (new nurses encouraged to apply).
- Leadership skills and ability to form relationships with residents, loved ones, and colleagues.
- Ability to perform physical tasks required for the role.
