Coleman Professional Services, Inc. is seeking a dedicated and compassionate individual to fill the position of Youth Behavioral Health Case Manager - Community Specialist.

About the Position

As a Youth Behavioral Health Case Manager - Community Specialist, you will provide short-term case management services to youth and their families in their natural environment, schools, and various community settings. Your primary goal will be to support the well-being and stability of these individuals by connecting them with necessary resources and services.

Duties and Responsibilities

  • Provide short-term, community-based case management to assigned families following a behavioral health crisis.
  • Develop and implement safety plans, increasing healthy coping skills for individuals and families.
  • Create treatment plans tailored to meet the unique needs of each family.
  • Assist with accessing supports, resources, and outpatient services that will help stabilize the crisis and keep them safe in the community.
  • Collaborate as part of an interdisciplinary team, fostering effective relationships with community professionals.

Qualifications and Requirements

  • Bachelor's degree or Master's degree in behavioral health or a related field.
  • Must be eligible for an NPI number and an Ohio Medicaid number.
  • Avoidance of employment opportunities without regard to race, color, religion, sex, national origin, sexual orientation, gender identity, disability, or protected veteran status.

Preferred Qualifications

  • Prior case management experience.
  • Understanding of child and adolescent development, behavioral health, and community resources.
  • Past or current experience in motivational interviewing, crisis intervention, and trauma-informed care.
