Launch Your Eye Care Services in Walmart Stores

As an Independent Optometrist leasing space inside our Walmart stores, you will take a leading role in patient care with respect to eye and vision care as well as general health and well-being. You will have the autonomy to establish your days of practice, hours of operation, patient schedule, and fee schedule, while keeping 100% of exam/professional fees.

You can opt into equipment/instrumentation and administrative and/or Opt-Tech support services (if state allows) for a fair market value. To be eligible for a lease agreement, you must meet the following minimum qualifications:

  • Hold a Doctor of Optometry (OD) degree from an accredited optometry school and a valid state optometry license, or be an Ophthalmologist (MD or DO) from an accredited school with a valid state ophthalmology license.
  • Have a General Liability Certificate and Professional Liability Insurance Certificate.
  • Provide a W-9.
