Position Overview:

The Clinical Registered Nurse (RN) plays a vital role in delivering both direct and indirect nursing care to participants within the PACE program. This role operates under the guidance of the Nurse Manager or their delegate while exercising independent judgment in complex clinical scenarios.

Key Responsibilities:

Assessment:

  • Gather patient information and complete necessary documentation.
  • Provide support as a backup for the Triage Nurse.
  • Conduct fall assessments in collaboration with physical therapy.
  • Perform initial, semi-annual, and annual assessments prior to team meetings.
  • Identify participant needs and establish priorities for referrals and follow-ups.
  • Interpret and document diagnostic results; execute advanced nursing assessments.
  • Conduct in-home evaluations for prospective and current participants.
Care Planning:
  • Actively participate in the Interdisciplinary Team, attending meetings and contributing to care planning.
  • Develop comprehensive care plans based on nursing processes, integrating input from other disciplines.
  • Engage patients and families in the development or revision of care plans.
  • Facilitate referrals to interdisciplinary support services.
Participant Care:
  • Administer treatments and procedures, including IV therapy, tube feeding, and wound care.
  • Manage medications, communicate needs, and coordinate with pharmacy services.
  • Provide education on medications and care delivery to participants and families.
  • Recognize emergency situations and initiate appropriate interventions.
  • Coordinate participant care with other healthcare professionals and assess the effectiveness of care measures.
  • Ensure proper inventory management of the medication room and maintain necessary logs.
  • Monitor hospitalizations and fulfill admission and discharge responsibilities.
  • Document care provided and any deviations from the care plan, reporting new infections as necessary.
Leadership:
  • Participate in clinic meetings and huddles.
  • Delegate nursing responsibilities appropriately using the nursing process.
  • Attend Nursing Facility Care Conferences as required.
  • Incorporate cost-effective practices into nursing care.
  • Identify and document unit issues, taking responsibility for resolution.
  • Engage in Quality Improvement processes actively.
  • Address ineffective patient care management, resolving issues and completing necessary documentation.
Qualifications:
  • Graduate from an accredited School of Nursing.
  • Current Registered Nurse license in the State of Michigan.
  • CPR certification is required.
  • Preferred: Three years of nursing experience in a healthcare setting.
  • Preferred: At least one year of experience working with elderly or frail populations.
