We are seeking a highly skilled and compassionate Physical Therapist to join our team at Upstream Rehabilitation in West Chester, Ohio.
About the RoleAs a Physical Therapist, you will be responsible for providing high-quality patient care and treatment plans to help patients recover from injuries and manage chronic conditions. You will work closely with our team of healthcare professionals to ensure seamless care and exceptional patient outcomes.
Responsibilities- Conduct thorough evaluations and assessments of patients to identify treatment needs
- Develop and implement personalized treatment plans to address patient goals and objectives
- Provide evidence-based physical therapy interventions to promote patient recovery and wellness
- Collaborate with healthcare teams to ensure comprehensive care and coordination
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care
- Graduate of an accredited Physical Therapy Program
- Current or pending state licensure
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
- Commitment to providing exceptional patient care and outcomes
