Job Summary

We are seeking a highly skilled Social Worker II to join our team at Houston Methodist. As a key member of our healthcare team, you will provide comprehensive case management and psychosocial assessments to patients and their families, ensuring seamless transitions and optimal outcomes.

Key Responsibilities
  • Conduct thorough psychosocial assessments to identify patient needs and develop effective treatment plans
  • Collaborate with healthcare teams to ensure comprehensive care and coordinate discharge planning
  • Provide education and support to patients and families on health-related issues and community resources
  • Develop and implement case management plans to address social, emotional, and financial barriers to care
  • Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements
  • Master's degree in Social Work from an accredited university
  • 3+ years of experience in social services, preferably in a healthcare setting
  • LCSW or LMSW licensure (with additional certification as required)
  • Strong communication and interpersonal skills, with ability to work effectively with diverse populations
  • Proficiency in Microsoft Office Suite and electronic medical records
