We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Northwest Healthcare Center. As a LPN, you will play a vital role in providing high-quality patient care and contributing to our mission of delivering exceptional healthcare services.
Responsibilities:
- Provide direct patient care, including administering medications, wound care, and vital sign monitoring
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes
- Participate in quality improvement initiatives to enhance patient satisfaction and clinical outcomes
Requirements:
- Current licensure as a Licensed Practical Nurse in the state of Maryland
- Minimum 1 year of experience in a healthcare setting, preferably in skilled nursing or rehabilitation
- Strong communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams
