HT Health is seeking a highly skilled Physical Therapist to join our team of healthcare professionals. Responsibilities
- Develop and implement individualized physical therapy plans to meet the unique needs of each client.
- Conduct thorough assessments and evaluations to identify client needs and goals.
- Provide hands-on physical therapy interventions, including exercises, stretches, and other modalities, to help clients achieve optimal mobility and function.
- Collaborate with other healthcare professionals, including nurses and social workers, to ensure comprehensive care and support for clients.
- Communicate effectively with clients, families, and caregivers to educate them on physical therapy techniques and provide guidance on home exercise programs.
- Document client progress and outcomes in a timely and accurate manner.
- Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
- Master's degree in Physical Therapy from an accredited program.
- Current licensure as a Physical Therapist in the state of practice.
- Minimum of 2 years of experience as a Physical Therapist, preferably in a home health or long-term care setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a strong commitment to providing high-quality patient care.
