BAYADA Home Health Care is a leading provider of home health care services, seeking a skilled Physical Therapist to join our team in North Wilkesboro, North Carolina. This full-time opportunity involves performing home visits and providing rehabilitative care to adult and geriatric clients in designated territories throughout Wilkes and Yadkin Counties.

Job Summary:

As a Home Health Physical Therapist with BAYADA, you will be an integral member of a multi-disciplinary healthcare team that provides skilled nursing and rehabilitative care to clients in their homes. Your expertise will play a vital role in helping our clients achieve their goals and regain independence.

About the Role:

  • Conduct home visits to assess client needs and develop individualized care plans
  • Perform diagnostic tests and measurements to evaluate client mobility, strength, and balance
  • Implement evidence-based interventions to promote wound healing and prevent further complications
  • Educate clients and family members on proper care techniques and provide guidance on maintaining independence
  • Accurately document client interactions and service delivery using state-of-the-art technology

Requirements:

  • A current North Carolina Physical Therapist license
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
  • Ability to work independently and manage time effectively
  • Strong interpersonal, organizational, and problem-solving skills
  • Solid computer skills, including experience with electronic medical records (EMR)
