Job Title: Cancer Center Social Worker MSW

About the Role:

We are seeking a highly skilled and compassionate Cancer Center Social Worker MSW to join our team at Memorial Hospital of Laramie County. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional patient care and support to individuals and families affected by cancer.

Key Responsibilities:

  • Conduct thorough psychosocial assessments to identify patients' needs and develop personalized care plans.
  • Provide emotional support and counseling to patients and their families, addressing concerns related to diagnosis, treatment, and end-of-life care.
  • Collaborate with healthcare providers to ensure seamless care coordination and communication.
  • Develop and implement discharge plans, ensuring patients receive necessary resources and support for a smooth transition.
  • Participate in multidisciplinary team meetings to discuss patient cases and develop comprehensive care plans.
  • Provide education and support to patients and families on cancer-related topics, including treatment options, symptom management, and coping strategies.
  • Document patient interactions, progress, and outcomes in a timely and accurate manner.

Requirements:

  • Master's degree in Social Work (MSW) from an accredited institution.
  • Licensed Clinical Social Worker (LCSW) certification in the State of Wyoming.
  • At least two years of experience in hospital social work or protective services.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong assessment and problem-solving skills, with the ability to think critically and make sound judgments.
