About the Role

We're seeking a dedicated and compassionate Physical Therapist to join our home health team. As a Home Health Physical Therapist, you'll play a vital role in designing and implementing treatment plans that restore patients' physical, social, and mental functions.

Your primary responsibilities will include:

  • Conducting comprehensive evaluations to identify patients' needs and develop personalized treatment plans.
  • Providing therapy interventions using evidence-based techniques and protocols.
  • Collaborating with interdisciplinary teams to ensure seamless care coordination.
  • Maintaining accurate and timely clinical documentation.

Requirements

To be successful in this role, you'll need:

  • A Bachelor's Degree or equivalent in Physical Therapy (some certifications may require a Master's Degree).
  • Current Physical Therapy licensure in the state of practice.
  • Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification.
  • Strong communication and interpersonal skills.
