Job Title: Assertive Community Treatment Team Leader

Segue, Inc. is a non-profit organization dedicated to providing quality mental health services to the citizens of Jackson & Hillsdale counties in Michigan. We are seeking a qualified Assertive Community Treatment Team Leader to join our team at our Hillsdale, MI facility.

Job Summary:

The Assertive Community Treatment Team Leader will lead a team of professionals in providing Assertive Community Treatment (ACT) services to individuals with chronic mental illness. This role requires clinical guidance, direction, and support to the team and the Impact program's consumers.

Responsibilities:
  1. Lead the ACT program, providing daily team meetings and coordinating activities.
  2. Offer clinical supervision and guidance to the team, directing appropriate intervention strategies and taking the lead in crises.
  3. May lead group sessions for consumers and provide therapy as needed.
  4. Direct monitoring of consumers released from the hospital for 30 days to reduce recidivism.
  5. Responsible for treatment planning meetings, developing treatment plans, and quarterly treatment monitoring reviews.
  6. Counsel clients individually, in family, or other small groups.
  7. Conduct 60-day new-hire and annual performance appraisals and participate in staff disciplinary actions.
  8. Monitor program activities to ensure compliance with applicable laws, regulations, CARF, and contract provisions.
  9. Keep the team and administration informed of important client or staff information.
  10. Work closely with local service providers to coordinate the delivery of services.
  11. Represent Segue in the community, including other public and private agencies.
  12. Conduct interviews to fill vacancies in the Impact program and orient new employees.
  13. Responsible for counseling and disciplining staff and participating in the termination of employees.
  14. Gather statistical information for QI and participate in the Professional Staff Organization (PSO).
  15. Arrange for and present cases to Behavioral Risk, PSO, Court/Judge, or other bodies.
  16. Consult for after-hours on-call staff regarding hospitalization and/or crisis home placement.
  17. Attend clinical supervision with the Clinical Director and as required to maintain licensure.
  18. Required to work additional hours or other duties as directed by Administration.
Requirements:
  • Master's degree in Psychology, Social Work, or related human services field.
  • Training and knowledge of mental health and community resources with two years of direct service experience.
  • Licensure in social work or psychology in the State of Michigan is required.
  • Certification in addiction through MCBAP is required.
  • Able to type 35 wpm with a high degree of accuracy; experience working with EMR preferred.
  • Knowledge of the Mental Health Code, Medicaid Standards, and CARF standards is required.
  • Must live within a 45-minute radius of the Hillsdale office to meet the on-call responsibilities.
