Supplemental Health Care is seeking a skilled Travel Nurse RN Home Health Case Manager to join our team. As a Travel Nurse RN Home Health Case Manager, you will be responsible for providing high-quality patient care and case management services to patients in their homes.

Job Summary:
  • Provide comprehensive case management services to patients in their homes, including assessment, planning, implementation, and evaluation of care.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Collaborate with healthcare providers, family members, and caregivers to ensure seamless care coordination and communication.
  • Conduct regular home visits to assess patient progress, provide education, and make adjustments to care plans as needed.
  • Document all patient interactions, assessments, and care plans in a timely and accurate manner.
Requirements:
  • Current Illinois RN License
  • Minimum of 1 year of recent home health nursing experience (private duty does not qualify)
  • OASIS/Medicare Certified Visits Experience
  • Electronic Charting experience
  • American Heart Association BLS
  • Valid U.S. Driver's License
