Nephrology Licensed Practical Nurse Job Description

WellSpan Health is seeking a skilled Licensed Practical Nurse (LPN) to join our Nephrology team in York, PA. As a key member of our healthcare team, you will provide high-quality patient care and support our patients with kidney-related health issues.

Job Summary

This LPN role is responsible for providing nursing care to patients with various kidney conditions, including chronic kidney disease, kidney failure, and dialysis. You will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring the best possible outcomes for our patients.

Responsibilities
  • Obtain patient history and gather information to develop a comprehensive understanding of each patient's needs.
  • Perform specialized procedures, examinations, and tests as ordered and aligned with the Ambulatory Clinical Scope Document.
  • Ensure all patient care areas are adequately stocked with unexpired supplies and medications.
  • Relay information and facilitate communication between the provider and the patient concerning patient care.
  • Evaluate patient's/caller's health status based upon presenting symptoms and follows defined protocols.
  • Tracks, reviews, and monitors the follow-up of diagnostic test results to ensure the physician/APP have verified results and patient has been notified.
  • Assists in the management of patient care between visits.
  • Completes necessary paperwork, prior authorizations, and/or scheduling of appointments for patients who are ordered diagnostic and therapeutic services.
  • Maintains patient records to ensure accurate documentation of the patient's medical treatment.
  • Adheres to Health Insurance Portability and Accountability Act (HIPAA) of 1996.
  • Documents and processes prescription requests and refills per Physician/APP order and or policy.
Requirements
  • Diploma Program Certificate of completion (degree or diploma) from an accredited LPN program.
  • 1 year Clinical nursing experience.
  • Licensed Practical Nurse Upon Hire Required.
  • Basic Life Support Upon Hire Required.
