Job Summary

We are seeking a skilled Nurse Practitioner to join our team at Ascension Health. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our medical staff to ensure the best possible outcomes for our patients.

Key Responsibilities
  • Provide direct patient care in accordance with established policies and procedures.
  • Implement and monitor patient care plans, monitoring patient condition and communicating with the healthcare team as needed.
  • Coordinate patient care with other healthcare professionals, including physicians and therapists.
  • Assess and coordinate patient discharge planning needs with the healthcare team.
Requirements
  • BLS Provider certification required within 1 month of hire date.
  • Registered Nurse licensure from the Indiana Board of Nursing required.
  • Diploma from an accredited school of nursing or required professional licensure at time of hire.
