About the Role

We are seeking a highly motivated and results-driven Community Account Executive to join our team at HarmonyCares. As a key member of our outreach team, you will be responsible for enrolling eligible patients into our HarmonyCares program.

Key Responsibilities
  • Develop and execute community outreach account plans for assigned geographies, including research on community provider practices and patient healthcare needs.
  • Conduct in-person visits to physician offices, hospitals, and skilled nursing facilities to educate community providers and engage patients into the HarmonyCares program.
  • Collect, analyze, and report patient outcomes and experiences to community physicians.
  • Conduct telephonic outreach to eligible patients or caregivers to enroll patients in the HarmonyCares program and support team engagement goals.
  • Collaborate closely with physician leaders to support community relationships and be the face of HarmonyCares in the community.
  • Meet or exceed established monthly engagement targets.
Requirements
  • College degree or 5 years of progressive career growth.
  • 3+ years of related experience in sales, account management, community outreach, or patient engagement in a medical/healthcare industry.
  • An understanding of value-based care and how it can impact patient outcomes.
  • Proficiency in Microsoft Excel, Word, and PowerPoint, with the ability to generate Excel reports.
  • A dynamic and engaging personality with excellent interpersonal, verbal, and written communication skills.
  • Detail-oriented, with the ability to think on your feet and respond with great follow-up.
  • Compassion, an interest in people, and a professional presentation.
  • Curious and a continuous learner, with a passion for solving problems and a get-it-done attitude.
