Position Overview

The Registered Nurse plays a crucial role in delivering high-quality care within our Medical Operations team. This position involves addressing patient inquiries, aiding in appointment scheduling, and utilizing clinical expertise and analytical skills to enhance patient health and well-being.

Key Responsibilities:
  • Clinical Practice: Assess, implement, and refine a personalized home care treatment strategy for participants. Observe changes in participants and modify the treatment plan as necessary. Deliver skilled nursing services as required.
  • Supervision and Training: Guide and oversee geriatric aides and LVNs in executing the home care treatment strategy. Educate and counsel participants and their families on care and safety protocols.
  • Interdisciplinary Collaboration: Actively engage in the interdisciplinary team by contributing clinical insights during planning and problem-solving sessions. Maintain effective communication with participants, their families, and team members.
  • Additional Duties: Perform other related tasks as assigned.
Qualifications:
  • Graduation from an accredited nursing program with either an associate or bachelor's degree in nursing.
  • Possession of a valid California Registered Nurse license. A California Public Health Nurse certificate is preferred.
  • Experience in public health nursing is advantageous.
  • A minimum of one year of experience working with elderly or vulnerable populations is required.
  • Ability to collaborate effectively within a multidisciplinary team.
  • Valid CA Driver's license and willingness to travel as needed.
  • Current CPR certification.
