Summary/Objective:

The HCBS Homes Support Specialist I is tasked with the execution and documentation of objectives and goals for the individuals in care. This role emphasizes advocacy for the individuals served, focusing on their personal choices and decision-making processes.


Essential Job Functions:

Requirements:

Physical Demands:

The physical demands outlined here are representative of those that must be met by an employee to successfully execute the essential functions of this role.

- Must remain alert during working hours.
- May assist in transferring individuals to/from wheelchair, chair, bed, toilet, vehicle, etc.
- May require lifting at times, up to 35 pounds. Lifting over 35 pounds necessitates the use of a two-person lift, mechanical lift, or assistive equipment.
- May participate in activities chosen by the consumer (e.g., church services, swimming, bowling, etc.).
- May require prolonged walking and standing for up to eight hours.
- May involve frequent bending, stooping, and stretching.
- May require driving for up to eight hours.
- May encounter various climatic conditions.
- May involve working with challenging emotional and physical behaviors (e.g., seizures, physical aggression, etc.).
- Must possess the ability to see objects/persons at a distance.
- Must have the ability to see close work such as typed or handwritten material.
- Must have the ability to hear conversations in both quiet and noisy environments.
- Must be able to determine the source of sounds.
- Must be able to differentiate among bells, buzzers, beeps, horns, etc.
- Must be able to communicate effectively through speech.
- Must be able to drive and transport individuals for service provision as necessary.
- Must remain alert during working hours, if applicable.

Required Qualifications:

- SCL - Minimum 16 years of age. HAB - Minimum 18 years of age.
- HAB – Minimum high school diploma or GED equivalent.
- Background checks with previous employers, references, law enforcement agencies, and others will be conducted and must be satisfactory.
- Valid driver's license and driver's record deemed acceptable by Imagine the Possibilities, Inc.
