California Correctional Health Care Services is seeking a skilled Registered Nurse to join our team in a correctional facility setting. As a Resource Registered Nurse, you will play a critical role in providing high-quality nursing care to patients participating in our Complete Care Model initiatives.
Key Responsibilities:- Organize and provide necessary nursing care for patients in accordance with applicable policies, procedures, regulations, and program requirements.
- Serve as the Care Manager for patients with chronic care management needs and collaborate with the primary care team to ensure timely and appropriate health care delivery.
- Assess patients for side effects, signs/symptoms of recent drug or alcohol use or withdrawal, mental health, and/or medical conditions that interfere with treatment.
- Facilitate recovery, physical health, and wellness, medication management groups, and promote therapeutic relationships.
- Participate in primary care meetings and Population Management sessions, collaborate with the Primary Care Nurse on care visits, care planning, medication management, and administer injections.
- Possession of a current license as a Registered Nurse in California.
- Ability to pass the examination for the position.
- Background Investigation clearance prior to being hired.
- Medical Clearance prior to being hired.
- Valid CA Board of Registered Nursing issued License.
