About the Role:
As a Home Health Physical Therapist with CenterWell Home Health, you will play a vital role in helping patients recover from injuries, illnesses, or surgeries, and regain their independence. Our team of dedicated professionals is committed to providing personalized, comprehensive care that addresses the physical, emotional, and social needs of our patients.
Key Responsibilities:
- Develop and implement individualized physical therapy plans to help patients achieve their goals and improve their quality of life.
- Conduct thorough assessments and evaluations to identify patients' strengths and weaknesses, and develop targeted interventions to address their needs.
- Collaborate with our interdisciplinary team, including nurses, occupational therapists, and social workers, to ensure seamless care coordination and optimal patient outcomes.
- Provide education and training to patients and their families on proper techniques and strategies for managing their conditions and promoting healthy lifestyles.
- Document patient progress and outcomes, and communicate effectively with our clinical team and patients' physicians to ensure continuity of care.
Requirements:
- Master's degree in Physical Therapy from an accredited program.
- Current licensure as a Physical Therapist in the state where you will be practicing.
- Minimum of one year of experience in home health or a related setting.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Valid driver's license and reliable transportation.
