Job Description

The Client Case Manager plays a crucial role in ensuring the safety and well-being of youth clients at Avery's Village. This position is responsible for providing social and service support, acting as a primary advocate and liaison, and coordinating with various agencies to meet client needs.


Key Responsibilities:
  • Supervision by Executive Director and/or Director of Programs
  • Serve as a role model in behavior and work ethic for staff and clients
  • Adhere to Avery's Village Employee Policies and Procedures manual
  • Complete required training
  • Manage case load for up to 16 youth clients
  • Liaison with Child Placement Agencies (KVC, St. Frances, TFI, Cornerstones of Care)
  • Ensure timely receipt of documentation for client placement
  • Coordinate admission of new clients
  • Conduct initial assessments of new clients
  • Maintain accurate case books for clients
  • Request Qualified Residential Treatment Program for clients
  • Arrange health examinations and medication/therapy appointments
  • Prepare weekly and monthly progress reports on clients
  • Develop, monitor, and update program treatment plans for each client
  • Create transition plans and discharge documentation for clients
  • Provide crisis intervention services
  • Arrange transportation for clients to appointments
  • Support clients with school and employment
  • Liaise with CPA case managers, therapists, schools, employees, other outside agencies, foster families, or family members
