As a Clinical Pharmacist Specialist in our Anticoagulation Clinic, you will play a vital role in optimizing patient outcomes by providing patient-directed medication dosing and education. Responsibilities

• Collaborate with referring providers to initiate, discontinue, or adjust anti-thrombotic therapy in accordance with current treatment guidelines and medical literature.

• Ensure seamless communication and coordination with the healthcare team to provide comprehensive patient care.

• Utilize Epic and other software systems to document interventions and activities accurately in the EMR.

• Provide effective information to varied audiences, including patients, healthcare professionals, and peers, by assessing and disseminating drug information and literature resources.

• Identify and engage in quality improvement activities to enhance patient care, medication use processes, and pharmacy operations.

Requirements

• A Doctor of Pharmacy (Pharm.D.) degree.

• Experience in a clinic setting, with a strong foundation in human relations, effective communication, and computer literacy skills.

• Current licensure to dispense controlled substances in the State of Michigan and a current, unrestricted Pharmacy license in the State of Michigan.

• Superior knowledge of medication therapeutics, clinical expertise in Pharmacy systems, and excellent verbal and written skills.

• Ability to work well with internal staff and the public, with sound judgment in handling confidential or sensitive issues and material.

• Must be able to develop and lead teams effectively.
