We are seeking a highly skilled Licensed Vocational Nurse to join our team as a Clinical Care Coordinator. In this role, you will provide direct patient care under the supervision of a registered nurse, following the plan of care and working collaboratively with the team to achieve positive patient outcomes.

Responsibilities:
- Implement the plan of care initiated by the registered nurse
- Provide accurate and timely documentation consistent with the plan of care
- Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care
- Participate in coordination of home health services and report identified needs to the registered nurse and/or Clinical Supervisor
- Use and/or prepare equipment and supplies effectively and efficiently adhering to aseptic technique
- Participate in personal and professional growth and development
- Perform other duties as assigned by the registered nurse

Requirements:
- Graduate of an accredited practical nurse or vocational nursing program, with three years nursing experience, preferably in community/home health or medical/surgical
- Currently licensed as a licensed practical nurse or licensed vocational nurse in the state, with valid CPR certification and compliance with accepted professional standards and practice

The estimated salary for this position is $65,000 - $85,000 per year, depending on experience.
