Job Summary:
SAC Health is seeking a Clinical Health Specialist to join our team. As a key member of our primary care team, you will provide integrated care services to patients, working closely with physicians, nurse practitioners, and other healthcare professionals. Your expertise in behavioral health will enable you to deliver high-quality patient care and support our mission to empower patients to live vibrant and healthy lives.

About the Role:
This is a full-time position requiring 4 days per week, 10 hours per day, Tuesday - Friday, 7:00am - 5:30pm. The salary for this position includes a geographical differential, and we offer an excellent benefits package effective on your first day. Responsibilities:
The successful candidate will be responsible for conducting assessments/screenings, providing therapeutic interventions, developing treatment plans, and coordinating case management activities. You will work closely with the medical team to meet the needs of individual patients, ensuring seamless care coordination. Additionally, you will participate in utilization review activities, track medical and behavioral health outcome measures, and represent SAC Health to external organizations as assigned.

Requirements:
To succeed in this role, you must possess a Master's Degree in Social Work (MSW) and hold an active Board of Behavioral Science License. A minimum of 1-2 years of licensed clinical experience is preferred, along with strong interpersonal skills, critical thinking abilities, empathy, cultural competence, emotional intelligence, adaptability, and communication skills. Fluency in English and Spanish is also highly valued.
