Job Description

\

The RN Hospice Specialty Registered Nurse ensures quality and safe delivery of hospice nursing services to patients in the home or assisted living setting.

\

This involves developing, implementing and evaluating the plan of care, making necessary revisions to address all problems in the plan of care.

\

The RN Hospice Nurse provides coordination of the plan of care, maintaining continuity of client care with other health professionals while identifying interdisciplinary care needs and making recommendations for additional home health care services.

\

Required Skills and Qualifications

\

A current RN License is required, with hospice AND/OR home care experience being preferred.

\

New graduates will not be considered.

\
