Job Summary: We are seeking a skilled Speech Language Pathologist to join our team as a Home Health Speech Language Pathologist in Pueblo, CO.
Key Responsibilities:
- Provide home-based speech therapy to patients with communication, swallowing, or cognitive difficulties due to illness or injury.
- Work as part of a home health team to develop and implement patient care plans.
- Conduct patient assessments, document progress, and ensure patient goals are met.
- Educate patients and families on therapy goals, safety practices, and self-care.
Requirements:
- Master's or Doctoral degree in Speech-Language Pathology and active SLP license in Colorado.
- Minimum of 1 year of direct client care speech therapy experience in the last 5 years, ideally in home healthcare.
- CPR Certification.
- Knowledge of state and federal home health regulations.
