We are seeking a skilled Licensed Social Worker to join our team at WorkStaff USA Staffing Agency LLC. As a Remote/Hybrid LCSW, you will provide crisis and triage management services to individuals, families, and groups.
Job SummaryThe successful candidate will be responsible for providing ambulatory behavioral health services, including evaluation, diagnosis, treatment planning, case management, and psychotherapeutic intervention. This role requires a strong understanding of crisis management and the ability to work in a fast-paced environment.
Key Responsibilities- Assist with the administration, development, implementation, and evaluation of social work programs and policies.
- Work as part of a team in the Call Center Advice Line to provide telephonic triage and assessment for members requesting behavioral health services.
- Provide telephonic crisis management for members seeking behavioral health services.
- Coordinate admissions and discharges for members requiring psychiatric admissions.
- Provide telephonic follow-up for post-hospital and ER discharges.
- Conduct clinical social work consultation, perform clinical diagnosis, risk assessments, and crisis interventions.
- Provide psychotherapy and counseling for individuals, families, groups, and/or Intensive Outpatient Programs.
- Interview members and evaluate their psychological and mental status, utilizing the latest Diagnostic and Statistical Manual of Mental Disorders (DSM) to diagnose patients.
- Assess urgent or high-risk clinical situations and make appropriate interventions.
- Coordinate referrals of patients to psychiatrists or other clinicians, assist with admissions to hospitals or other specialized facilities, and plan and provide services in the appropriate level of care.
- Perform case management, coordination, and monitoring of social work service plans in areas of personal, social, or economic resources, conditions, or problems.
- Assess treatment and prevention of psychosocial dysfunction, disability, or impairment, including emotional and mental disorders.
- Conduct preparation and evaluation of psychosocial assessments and development of social work service plans.
- Help patients enhance or restore their capacities for personal and social functioning and prevent and control social problems.
- Gather information and identify resource and referral services for patient care, educate clients regarding behavioral health services, and assist members to use services.
- Participate in or assist with therapeutic and educational group programs, evaluate outcomes and effects of treatment for individual patients and programs, and research, collect, and record data and information relating to identification and treatment of behavioral and interpersonal problems of patients.
- Collaborate and consult with physicians and other interdisciplinary clinical team members to support assessments and decision-making, review difficult cases or quality assurance concerns with clinical teams, and act as a consultant to team members and others in the organization.
- Maintain a high level of clinical expertise, be knowledgeable of current clinical conditions, methods, medications, theoretical frameworks, and interventions, and utilize best practices in delivery of behavioral health services in accordance with professional and clinical guidelines.
- Maintain clinical records by documenting clinical assessments, treatment plans, interventions, and all pertinent information in patient medical records according to Regional and departmental guidelines.
- Participate in clinical team meetings, departmental planning, and quality management activities.
- Assess patients in Emergency Departments on an as-needed basis in service areas where Kaiser Permanente has privileges to provide mental health assessments and crisis intervention.
- Perform other clinical duties as assigned.
- Masters Degree in Social Work from an accredited college or university, passing score on the clinical level national board examination, and supervised clinical social work training, as required by Hawaii Revised Statutes (HRS) 467E-7.

- National Provider Identifier required at hire
- Basic Life Support required at hire from American Heart Association
- Knowledge and experience in conducting assessments and diagnostic evaluations using the latest Diagnostic and Statistical Manual of Mental Disorders (DSM).
- Demonstrated knowledge of and skill in adaptability, change management, conflict resolution, decision-making, group process facilitation, influence, interpersonal relations, oral communication, problem-solving, quality management, teamwork, and written communication.
- Taxonomy code required at time of hire.
- Note: This position requires successful completion of credentialing process upon hire.
- Demonstrated knowledge and skill in documentation according to national accreditation guidelines.
- Experience in providing behavioral health services in a primary care setting.
- Experience in providing services to patients with addictions issues.
- Experience in providing Consultation/liaison Services to emergency departments.
- Experience in providing evidence-based treatment, including conducting individual and group therapy, intensive outpatient treatment, and/or partial hospitalization programs for children and/or adults, including people with serious/chronic mental illness.
- Experience in writing treatment plans.
- Position requires working over the phone in highly stressful situations and with interruptions.
