As a LPN, you will provide one-to-one patient care in a home environment, executing physicians' prescribed plans of care and documenting care provided in our system of record.
Responsibilities:
- Provide high-quality patient care in a home environment, adhering to physicians' prescribed plans of care.
- Document patient care and progress in our system of record, ensuring accurate and timely reporting.
- Administer prescribed medication, treatments, and therapies as ordered by physicians.
- Conduct patient assessments, coordinate care, and provide health education to patients and their families.
- Collaborate with our interdisciplinary team to ensure seamless care coordination and patient outcomes.
Requirements:
- Current LPN licensure in the state where you will practice.
- At least 6 months of hands-on nursing experience.
- Current CPR certification.
- Ability to provide in-home nursing care to infants, children, and adolescents.
