Job Summary:

As a Home Care Aide with Kaiser Permanente, you will provide personal care and support services to patients in their homes. You will work as a member of the hospice/home health team in providing care within the guidelines of the plan of care to best meet the needs of the patient. You will report directly to the RN case manager or PT/OT/ST.

Key Responsibilities:

  • Assist patients with and provide personal care, including bathing, skin care, oral hygiene, foot care, toileting/elimination, and clean dressing changes as directed by the RN or PT/OT/ST.
  • Assist patients to carry out their prescribed therapeutic regimen under the supervision of the case manager, including active/assistive range of motion and positioning, safe transfer techniques and ambulation, and active/assistive exercises.
  • Provide household chore services essential to healthcare at home, including meal preparation, linen change, personal laundry of the patient, and dishwashing.
  • Observe and document patients' condition, including weight, vital signs (temperature, pulse, respiration, and blood pressure) and care or service furnished.
  • Recognize and handle emergency situations as directed until help arrives.
  • Understand and apply basic infection control procedures and maintain a clean, safe, and healthy environment for patients.
  • Document timely and accurately in the clinical record according to hospice policies.
  • Reinforce education provided by the Case Manager to patients and caregivers.
  • Provide emotional support and compassionate care to the patient and family unit, demonstrating listening skills and offering encouragement when appropriate.
  • Review the medical record to validate assignment and HCA orders.
  • Participate in performance improvement activities and assist with development and implementation of departments' workplan.
  • Prepare weekly schedules of patient visits for self and keep the Supervisor or Intake Coordinator apprised of schedule changes and openings for other added visits.
  • Rotate weekends and holidays with other HCAs as assigned.
  • Participate in interdisciplinary team conferences and attend continuing education and in-service training programs to keep current on patient care techniques/needs.
Requirements:
  • Minimum of one (1) year aide experience providing direct patient care, including successful completion of a training program in the area of specialty, if applicable.
  • Completion of a nurse aide program or equivalent related experience.
  • Driver's License (Hawaii) and Basic Life Support from the American Heart Association.
Preferred Qualifications:
  • Experience in and ability to support postmortem care.
  • Exhibits compassion in helping to care for patients and families who may be experiencing grief and loss.
  • Great interpersonal, communication, and collaboration abilities, as well as an ability to provide compassionate end-of-life care.
  • Exceptional ability to manage stress and emotional challenges.
  • Competency in electronic patient and hospice management systems, such as Epic.
