Job Opportunity:
We are seeking a Part-time Geriatric Rehabilitation Therapist to join our team at HealthMark Rehabilitation in Fort Worth, Texas.

About Us:
HealthMark Rehabilitation is an in-house therapy company serving Skilled Nursing Facility patients and residents. We prioritize delivering premier healthcare services through dedicated and compassionate employees, accommodating individualized care needs of our patients.

Responsibilities:
- Provide therapy services in accordance with physician's orders and professional practices.
- Develop overall therapy plans of care, including treatment, medical diagnosis, duration, frequency, short-term and long-term goals that support discharge plans, following established guidelines and standards of practice.
- Perform skilled treatments according to established plans of care.
- Document observations, patient responses to interventions, degree of progress, and notable changes in condition within specific timeframes.
- Complete all required documentation timely, adhering to state and federal regulations, policies, and guidelines.
- Maintain the rehab gym equipment and treatment area in a safe manner using good infection prevention methods.

Requirements:
- Hold Texas license/certification for appropriate discipline (CCC-SLP).
- Possess strong oral and written communication skills, organizational, and analytical skills.
- Ability to function effectively in multi-faceted systems, interact with staff and customers, demonstrate time management, and organizational skills.
- Utilize independent and effective clinical judgment, problem-solving, and decision-making skills.
- Meet physical requirements to deliver responsibilities.
- Experience in a geriatric rehabilitation setting preferred.
- Successful completion of hiring process/background check.
