We are seeking a highly skilled Nurse Practitioner to join our family medicine department at Northwestern Memorial Healthcare. As an Advanced Practice Provider, you will play a vital role in delivering high-quality patient care and supporting the needs of our practice.
Key Responsibilities- Conduct comprehensive appraisals of patients' health status, taking health histories and performing physical examinations.
- Provide direct clinical care to patients, including prescribing medications and ordering laboratory tests and procedures.
- Evaluate patients to determine their need for further health care and provide comprehensive teaching to patients and families.
- Document all care and education provided in the medical record.
- Refer patients to other disciplines as appropriate and consult with other disciplines in areas of expertise.
- Advanced Practice Provider licensure and certification.
- Experience in family medicine or a related field.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
