Innovation Home Health Care LLC is seeking a skilled Physical Therapist to join our team in providing high-quality home health care services to patients in need. As a Physical Therapist, you will play a vital role in helping patients recover from injuries, manage chronic conditions, and maintain their overall health and well-being.
Responsibilities- Conduct thorough assessments of patients' physical abilities and develop personalized plans of care to address their unique needs.
- Provide one-on-one therapy sessions to patients in their homes, focusing on exercises, stretches, and other interventions to improve their mobility, strength, and function.
- Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care and coordination of services.
- Document patient progress and outcomes, and communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
- Stay up-to-date with the latest evidence-based practices and guidelines in physical therapy, and participate in ongoing education and professional development to enhance your skills and knowledge.
- Current Florida Physical Therapy License
- Minimum of 1 year of experience as a Physical Therapist in home health care
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
- Ability to lift, transfer, and position patients as needed, with or without reasonable accommodation
- Flexibility to work a variable schedule, including evenings and weekends, as needed
- Competitive hourly rate of $100.00
