About the Role

We are seeking a highly skilled Occupational Therapist to join our team at EmpowerMe Wellness. As a key member of our care coordination program, you will have the opportunity to practice at the top of your license while expanding your skills in care coordination.

Responsibilities
  • Provide occupational therapy services to older adults, including assessments, treatment planning, and therapeutic interventions.
  • Support care coordination efforts by conducting regular wellness visits, communicating with patients' healthcare providers, and assisting with telehealth visits.
  • Maintain professional practices and ethical standards in accordance with company policies and clinical initiatives.
  • Contribute to program development, quality improvement, and problem-solving initiatives.
  • Optimize each patient's functional well-being and satisfaction.

Requirements
  • Completion of an accredited Occupational Therapy program.
  • Current state licensure in Occupational Therapy.
  • Excellent verbal and written communication skills.
