Primary Care Outpatient APP

Join our team of dedicated healthcare professionals as a Primary Care Outpatient APP. As a key member of our collaborative care team, you will provide high-quality, patient-centered care to our patients.
Key Responsibilities- Manage patient populations in a collaborative team environment, working closely with physicians and other healthcare professionals.
- Diagnose and treat acute and chronic conditions, including diabetes, hypertension, and kidney disease.
- Order and interpret diagnostic lab work, x-rays, and EKGs.
- Prescribe medications and other treatments as needed.
- Counsel patients on disease conditions and treatment options.
- Work collaboratively with the care team to provide comprehensive, coordinated care.
- Graduate of a PA/NP program.
- Current licensure through the Georgia Board of Medical Examiners.
- Current prescriptive authority from the Georgia Board of Medical Examiners.
- Ability to lift up to 50 pounds and perform other physical demands of the job.
