Job Summary
We are seeking a skilled Post-Acute Care Physician to join our team at Physician Empire. This is a full-time opportunity that will provide patient care within a high-volume, quality-focused, and established Post-Acute ACO Model of Care.
Key Responsibilities
- Provide seamless transitions in care and improved quality outcomes in Skilled Nursing Facilities
- Work in an integrated, coordinated, and collaborative environment to deliver high-quality patient care
- Be supported by an Advanced Practice Practitioner and a team of experienced healthcare professionals
- Board Certification in Family Medicine or Internal Medicine with a specialty in Geriatrics is preferred
- Experience in Long Term Care and Acute Care is essential
- Experience as a Hospitalist and/or SNFist is a plus
- Hold a valid MA License
- Competitive guaranteed base salary and compensation package, including malpractice coverage of $2m/$6m
