We are seeking a Part-Time Registered Nurse to join our team at Synergy Medical Services LLC. As a Home Health Registered Nurse, you will be responsible for creating and managing personalized care plans for patients in their homes. This role requires a strong attention to detail, excellent communication skills, and a passion for delivering high-quality patient care.
Responsibilities- Provide comprehensive care and support to patients in their homes, including administering medications, monitoring patient conditions, and documenting changes.
- Coordinate with other healthcare professionals to assess, plan, implement, or evaluate patient care plans.
- Develop and maintain accurate records of patient care, including observations and discussions with supervisors.
- Collaborate with patients and their families to ensure seamless care transitions and address any concerns or questions.
- Currently licensed Registered Nurse (RN) in the state of employment.
- Minimum of 1 year experience as a Registered Nurse in Home Health care.
- Previous experience with OASIS and Medicare guidelines/COPs preferred.
- Excellent interpersonal and client care skills.
- Valid driver's license and reliable transportation.
- First aid/CPR certification required.
