Physical Therapist Opportunity

Revival Home Health & Hospice is seeking a skilled Physical Therapist to join our team in Annandale, VA. As a Physical Therapist, you will have the opportunity to provide one-on-one care to patients in their homes, while working as part of an interdisciplinary team to deliver individualized care plans.

About the Role

As a Physical Therapist, you will be responsible for performing physical therapy evaluations and treatments, completing comprehensive evaluations, and developing treatment plans to help patients achieve their maximum level of independence and function. You will also communicate with the treatment team, physician, and patient's family members as needed.

Responsibilities

  • Perform physical therapy evaluations and treatments based on physician orders
  • Complete comprehensive evaluations using appropriate evaluation and assessment tools and procedures
  • Review patient records for current diagnosis, past medical history, precautions, and contraindications
  • Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and function
  • Regularly re-assess the effectiveness of treatment plans, progress of patients toward goals, and the need for modifying goals or treatment
  • Communicate patient needs and progress to the treatment team, physician, person receiving services, family members, as applicable
  • Consult with medical providers regarding patient evaluation and treatment, progress of patients, and discharge from treatment
  • Maintain and submit evaluation, treatment, and billing documentation in the format and timelines required by the company, facility, funding source, local, state, and federal regulations
  • Adhere to standards of the company's Code of Business Conduct, policies, Health Insurance Portability Accountability Act, and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal laws
  • Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act
  • Attend, participate in, and/or conduct internal staff development programs and meetings; obtain continuing education as required by company policy and regulations; maintain and enhance clinical practice skills


Qualifications

  • Graduate of a Physical Therapist Education program approved by CAPTE
  • Current Physical Therapist licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire
  • Two (2) years of experience in adult physical therapy, home health experience preferred
  • Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills; physical therapy skills per competency checklist
