Job Overview:

We are looking for a compassionate and experienced nurse to provide high-quality care to our residents. As a Geriatric Nurse Specialist, you will be responsible for assessing patient needs, developing care plans, and delivering exceptional patient outcomes.

Key Responsibilities:

  • Provide individualized nursing care to residents in accordance with their unique needs and preferences
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes
  • Maintain accurate and up-to-date medical records, ensuring compliance with regulatory requirements
  • Participate in ongoing education and professional development to stay current on best practices in gerontological nursing

Requirements:

To succeed in this role, you must possess a strong foundation in gerontological nursing, excellent communication skills, and the ability to work effectively in a fast-paced environment. A valid RN license and LTC certification are required.
