We are seeking a skilled Licensed Practical/Vocational Nurse to join our team in providing high-quality home health services. As a Field LVN, you will be responsible for delivering direct patient care under the supervision of a Registered Nurse. Your responsibilities will include implementing the plan of care, providing treatments, and working collaboratively with the healthcare team to achieve positive patient outcomes.

Responsibilities:

  • ">
  • Provide direct patient care as defined by the state Nurse Practice Act and agency policies.">
  • Implement the plan of care initiated by the Registered Nurse.">
  • Document patient information accurately and timely in accordance with the plan of care.">
  • Assess and provide education to patients and their caregivers on disease management and self-care.">
  • Participate in the coordination of home health services and communicate effectively with the healthcare team.">
  • Follow infection control protocols and safety guidelines.">
  • Utilize equipment and supplies efficiently.">
  • Participate in ongoing professional development and growth.">

Requirements:

  • ">
  • Current unrestricted license in the state of Texas as a practical nurse or licensed vocational nurse.">
  • BLS/CPR certification.">
  • 2+ years of clinical experience preferably in a community home health or medical/surgical setting.">
  • Access to reliable transportation for client visits.">
  • Texas Driver's license.">
  • Demonstrated ability to lift and transfer patients.">
  • IV Certification (optional).">
  • Wound Care Certification (optional).">
