Medical Care Coordinator

About the Role
This position is open to experienced nurses and new graduates alike. As a registered nurse, you will be responsible for coordinating patient care, educating patients and families, and transitioning patients to other levels of care. You will work collaboratively with other healthcare professionals to ensure that patients receive the best possible care. If you are a new graduate, you will have the opportunity to participate in our Nurse Residency Program, which provides valuable training and support as you begin your nursing career.
Key Responsibilities
- Coordinate patient care, including direct patient care, patient education, and transitions of care
- Work collaboratively with other healthcare professionals to ensure high-quality patient care
- Participate in quality improvement efforts to continuously improve patient care
Qualifications
- Current RN license by applicable state requirements
- Basic Life Support for Health Care Providers certification from American Heart Association or American Red Cross
- Additional certifications or training may be required depending on the work area
Salary and Benefits
The estimated salary for this position is $ $96,000 - $120,000 per year, depending on experience and qualifications. We offer a comprehensive benefits package, including health insurance, retirement savings plan, and paid time off.
