Key Responsibilities:
- Conduct medical nutrition therapy assessments for residents, addressing both acute and chronic health conditions.
- Provide education on therapeutic diets and nutrition interventions to enhance residents' health outcomes and quality of life.
- Collaborate with nursing staff, dietary managers, speech language pathologists, physicians, and nurse practitioners to deliver tailored nutritional therapy, including therapeutic diets and oral nutrition supplements.
- Monitor residents experiencing weight fluctuations, enteral feeding needs, dialysis, and wound care.
- Participate in resident review meetings to discuss high-risk cases.
- Perform nutrition assessments for a diverse patient population, managing the development and implementation of nutrition intervention plans in line with the Nutrition Care Process.
- Offer nutrition counseling tailored to individual learning needs, considering age-specific and cultural factors.
- Ensure compliance with food safety and sanitation standards while providing education on these topics.
- Maintain accurate documentation of nutrition care plans in patients' medical records, summarizing assessments, diagnoses, and progress towards treatment goals.

Qualifications:
- Bachelor's degree required; Master's degree preferred in Dietetics or a related field.
- Credential as a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN).
- Valid Illinois state licensure as a Dietitian.
- Familiarity with PointClickCare and MDS processes is advantageous.
