Job Summary

We are seeking a Registered Nurse (RN) Case Manager to join our team. As a key member of our care team, you will be responsible for performing visits and coordinating client care, serving as the point of contact for all disciplines involved in patient care, and overseeing the frequency of visits for each episode.

Responsibilities
  • Perform accurate OASIS collection and ensure the medication profile remains current
  • Consult with physicians and office staff as needed to provide detailed information about patient care
  • Attend weekly case conferences and monthly case manager meetings
  • Develop and implement individualized care plans to meet the unique needs of each patient
Qualifications
  • Current licensure as a Registered Nurse (RN) in the state of employment
  • A minimum of one year of clinical experience
  • Demonstrated knowledge and skill in current nursing practice
  • A valid state driver's license and automobile liability insurance
