Key Responsibilities- Provide primary care services to patients, including health promotion, disease prevention, and management of acute and chronic conditions.
- Conduct thorough physical exams, diagnose illnesses, and develop treatment plans to meet the unique needs of each patient.
- Prescribe medications and order diagnostic tests as necessary to ensure accurate diagnoses and effective treatment.
- Collaborate with other healthcare professionals to coordinate patient care and ensure seamless transitions between providers.
- Stay up-to-date with the latest medical research and best practices to provide evidence-based care and improve patient outcomes.
- Master's degree in Nursing or Physician Assistant Studies from an accredited program.
- Active, unrestricted license to practice as a Nurse Practitioner or Physician Assistant in the state of Texas.
- At least one year of experience in adult medicine as a Nurse Practitioner or Physician Assistant.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong analytical and problem-solving skills, with the ability to make sound judgments and decisions in complex situations.
- Bilingual (English and Spanish) or proficiency in another language.
- Knowledge of HEDIS measures and quality indicators.
- Experience managing Medicare Advantage panels and understanding of best practices in coordinated care environments.
