About the Role

BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team as a Home Health Care Specialist. As a Physical Therapist in Home Health, you will play a vital role in providing rehabilitative care and skilled nursing to clients in their homes.

Responsibilities
  • Conduct home visits to clients in designated geographic territories to assess their physical therapy needs.
  • Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
  • Develop and implement individualized care plans to improve clients' physical function and independence.
  • Continuously assess and revise care plans to ensure optimal outcomes.
  • Educate clients and their families on rehabilitative care and activities to promote healthy living.
  • Accurately document client care management and services provided.
Qualifications
  • A current Pennsylvania Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
