Key Responsibilities
As a patient navigator, you will:
1. Serve as a liaison among management, administration, physicians, managed care organizations, community groups, and other stakeholders.
2. Coordinate treatment efforts and promote quality improvement initiatives.
3. Collect data, monitor outcomes, and provide recommendations for process enhancements.
4. Offer referrals and support for follow-up care for patients lacking insurance or underinsured status.
Qualifications
Licensure / Certification / Registration:
  • Registered Nurse credential from the Wisconsin Board of Nursing required prior to hire or transfer. Licensure from the Wisconsin Board of Nursing or current home state license if considered multi-state/Compact State. BLS Provider preferred.

Education:
  • Diploma from an accredited nursing school or required professional licensure at the time of hire.

Preferred Qualifications
  • Degree in Nursing
  • Active RN Licensure
  • 3-5 years of experience, with 1-2 years in primary care preferred
  • BLS Certification required

Core Competencies
Maintain confidentiality of patient information in accordance with established policies and regulations such as HIPAA. Ability to work independently and apply clinical judgment in interactions with physicians, patients, and their families. Strong organizational and time management skills, with the ability to prioritize multiple tasks effectively. Understanding of scope of practice boundaries, with a willingness to seek guidance from appropriate resources. Proficient in computer skills and electronic medical records. Ability to collaborate as part of a team and work harmoniously with colleagues and providers.
Essential Functions
1. Deliver direct patient care.
2. Conduct Medicare Annual Wellness visits, coordinate patient care, provide education, and communicate effectively with patients, providers, and care teams.
3. Advocate for patients, addressing their concerns proactively.
4. Ensure documentation and billing of services comply with CMS guidelines and correct coding standards.
5. Maintain confidentiality of patient information as per established policies and regulations.
6. Conduct pre-visit planning to ensure appropriate follow-up and documentation of preventive screenings.
7. Understand population health management and ensure Medicare patients are managed within ACO program guidelines.
8. Familiarity with preventive care guidelines and make recommendations to patients accordingly.
9. Knowledge of community resources and assist patients in accessing those resources.
10. Perform other duties as assigned.
